Field Geriatric Care Manager -  LVN, RN and LSW 
(Part-time, Per Diem, Flexible Schedule)

Position Description: 

As a Field Geriatric Care Manager you will visit our clients in their home or at hospital and conduct a holistic assessment that will help you identify gaps in care and create a care plan and implement strategies for care. You will guide and facilitate interaction with resources appropriate for their care and well-being. You work in collaboration with a multi-disciplinary team, employing a variety of strategies, approaches and techniques to enable the client to manage their health condition through self-management education, their living environment and psychosocial status. 

Geriatric Care Management is defined as professional consulting which assists clients and their responsible parties with decisions related to aging, available services and long term care planning. Our objective is to assist our clients and their responsible parties in managing and navigating the issues they will face as they age. The care manager will assess each client and family situation by evaluating the client’s level of functioning, their health, their emotional state, their finances and their legal needs in an effort to enhance their overall quality of life. 

Key Competencies & Qualifications:

  • Must be a licensed RN, LVN or Social Worker. Experience in medical and long term care settings and care coordination preferred.
  • 2+ years experience in home health or hospice a plus.
  • 3+ years experience of care or case management or home health or hospice.
  • Certified Geriatric Care Manager a plus and carries a hiring bonus.

  • HIPAA certified. Respectful and compliance of company and client confidentiality.
  • Ensure that inappropriate placements, duplication of services, and unnecessary hospitalizations are avoided. 
  • Experience in resources regularly utilized by older adults (i.e. Medicare, Medicaid)
  • Bi-lingual: English/Spanish – Superior spoken and written communication. 
  • This position allows for a flexible schedule. You control your own appointments, thus efficient and effective time management and organizational skills are imperative.
  • Reliable transportation with current vehicle insurance, valid driver’s license.
  • Recent criminal background check and drug screening.
  • Physical Abilities: Must be able to sit for long periods, stoop, lift, drive and bend effectively for level required to perform job duties
  • Ability to travel 30 to 40 miles radius of home address.
  • Excellent computer and internet navigation skills.
  • Must have locked home office or separate room with locked door that can be used as a home office to ensure continuous privacy when you work on paperwork.

  • Develop, monitor and manage the care plan. 
  • Screen, arrange, and monitor all services to optimize benefit to the client. 
  • Review financial, legal, or medical issues and offer referrals to specialists. 
  • Identifying resources to address clients’ health, daily living, and safety needs. 
  • Assist with determining the level of care needed and arranging appropriate facility or in-home care. 
  • Monitoring clients’ health status and well-being. 
  • Provide crisis intervention. 
  • Act as a liaison to responsible parties and related professionals, to keep them informed of the client's situation. 
  • Provide counseling and support to client and their families. 
  • Educating the client and/or family on aging issues, health, and life choices. 
  • Conduct ongoing face to face outreach or telephonic with providers as well as private, non-profit, and governmental agencies and other members of the client’s care team.
  • Communicate effectively and timely with all involved participants. 
  • Complete documentation requirements accurately and timely. 
  • Work collaboratively with staff to ensure appropriate intervention implementation with advocacy organizations and healthcare facilities. 
  • Manage and maintain all assessment data in appropriate software applications.
  • Comply with all reporting requirements as defined by company.
  • Participate in staff and training meetings, community, and professional educational. 
  • Provide consumer education. 
  • Promote company to all referral sources, vendors, etc. 
  • Participate in marketing efforts, education of the public, cultivating clients, and securing new clients. 
  • Demonstrates competency in core care management areas outlined by the company. 
  • Complete additional tasks as requested by supervisor.
